1. Acknowledgment and Assumption of Risk

I, the undersigned parent or legal guardian of the above-named minor (“Participant”), understand and acknowledge that participation in any and all activities organized by Knights Football Club (“KFC”), including but not limited to: soccer camps, clinics, private training, one-on-one sessions, and group training (collectively, “Activities”), involves inherent risks.

These risks may include, but are not limited to: physical injury, illness, accidents, equipment failure, field hazards, weather conditions, and other hazards associated with physical activity and outdoor sports.

I understand that these risks can result in serious injury or even death, and I voluntarily allow my child to participate fully in these Activities, fully assuming all risks involved.

2. Waiver and Release of Liability

In consideration of my child’s participation, I, for myself and on behalf of the Participant and our respective heirs, assigns, and representatives, hereby waive, release, discharge, and hold harmless Knights Football Club, its directors, officers, employees, coaches, trainers, volunteers, affiliates, and all associated personnel from any and all liability, claims, demands, losses, or damages caused or alleged to be caused, in whole or in part, by the negligence or misconduct of the Club or otherwise arising from participation in the Activities.

3. Medical Treatment & Insurance Responsibility

I certify that my child is in good health and physically able to participate in all Knights Football Club Activities. In the event of an emergency, I authorize Knights Football Club staff to seek emergency medical treatment for my child.

I understand and agree that any medical expenses or insurance claims resulting from injury, accident, or illness are solely my responsibility and will be covered through my personal insurance policy.

I acknowledge that Knights Football Club does not carry or provide health or accident insurance for participants.

4. Photo & Video Release (Optional)

I grant permission for my child’s photo, video, or likeness to be used for promotional purposes by Knights Football Club, including but not limited to: website, social media, flyers, and newsletters.

5. Refund and Credit Policy – New York State Clause

I understand and agree that all payments made to Knights Football Club are non-refundable. If my child misses sessions or voluntarily withdraws from participation for any reason, I will not be entitled to a refund.

However, credit for missed sessions may be issued at the sole discretion of Knights Football Club and must be used within the same seasonal calendar year.

This policy is in compliance with New York State laws regarding private youth programs and ensures fairness and consistency for all participants.

6. Agreement and Signature

By signing my signature during registration, I confirm that I have read and understood this Waiver and Release of Liability in full, and I agree to all its terms freely and voluntarily.
